Helping the community find affordable and stable housing
Housing Stabilization Services is a brand-new housing service for all eligible Minnesotans created by the Department of Human Services. The service is designed to help folks living with disabilities to have the person-centered assistance they need to be successful in their housing endeavors – whether that be moving across the street, across the state, or even finding resources to help prevent homelessness. Housing Stabilization Services is comprised of three separate pieces, all of which offer a unique person-centered housing service: Transition, Sustaining, and Housing Consultation.
All Minnesotan adults on Medicare who are experiencing housing instability are eligible to receive Housing Stabilization Services. The eligibility criteria, as defined by the Department of Human Services, is that the person must:
Be on Medical Assistance (MA).
Be 18 years or older.
A person with disability
Age 65 or older
Be experiencing housing instability, homeless and or at risk of homelessness
Have a documented disability or disabling condition such as (CADI, DD, DI, EW, OR CAC waiver)
